APPLICANT'S STATEMENT

I authorize an investigation of all statements contained in this application and I agree to sign the Company’s form to authorize the company to perform a complete background check. I understand that any misrepresentation, omission of facts called for, criminal history, bad credit, and unethical or immoral acts may result in rejection of this application or dismissal from work. I understand that the company maintains an at-will work relationship with all workers. This means that no worker is contracted for a specific period of time and that all workers are contracted at the will of the company and are subject to termination at any time, for any reason, with or without cause or notice. At the same time, workers may terminate their work at any time, with or without notice.

I understand that no one at the company has the authority to enter into any work contract on behalf of the company except the President and/or Owner and any such contract must be in writing in order to be enforceable against the company.
